FieldName;LabelFieldName;Label. If Label is missing then Name = Label. dbNameMap cannot be used to provide the Caption of the user fields. Instead of editing a DB where the field names and data types are defined, User defined input fields must provide the names and labels, The names will be the URL variable. See example code.
